Monessen, Pennsylvania — A 53-year-old worker was killed Sunday after becoming pinned between two pieces of heavy-duty machinery at a coke plant in western Pennsylvania, authorities said.
Jeffrey Smith, a resident of Newell, Pennsylvania, was working at the Cleveland-Cliffs Coke Plant in Monessen when the fatal workplace accident occurred on Sept. 27.
According to a release from the Westmoreland County coroner’s office, Smith became trapped between “two door machines” at the facility. He was pronounced dead at 5:55 p.m. local time.
The circumstances surrounding exactly how the machinery accident occurred remain under investigation.
OSHA Is Investigating the Fatal Accident
The Occupational Safety and Health Administration (OSHA) is investigating Smith’s death as authorities work to determine what happened before he became trapped between the machines.

“It’s a blow. It’s a blow to the community,”
Yuhasz said. He noted that the facility had gone a significant amount of time without an incident of this nature.
“We’ve gone, I don’t know how long, without an incident at that facility. And it may never happen again. So, all we can do is offer help if any incidents do occur and just take it day by day.”
His comments reflected the impact of the death on workers and residents in a community closely connected to the industrial facility.
The investigation remains ongoing, and authorities have not indicated whether any violations or other contributing factors were identified.
Cleveland-Cliffs Confirms Employee’s Death

“We are deeply saddened by this incident, and our thoughts are with our employee’s family, friends, and coworkers,”
The company spokesperson added that the incident was being investigated by Cleveland-Cliffs, the United Steelworkers and local authorities.
